Applications and Forms

Below is a listing of applications and forms available to download from Campus Activities and Orientation (CAO). If you have any questions or need additional information, contact CAO at cao@augsburg.edu or 612-330-1418.

Funding & Grants

• Budget Request Form — This form is for qualifying student organizations to apply to become commissioned and submit their budget requests for the 2012-2013 academic year.
  
  
• Fundraising Permit — Student organizations must complete this form before fundraising anywhere on Augsburg College property.
  
  
• Grant Request Form (Day Government) — Student organizations should complete this online form to apply for grant money from Augsburg Day Student Government.
  
  

Locker Reservations & Office/Workstation Keys

• Auggie's Nest Key Request Form — This form is to request a key for an Auggie's Nest office: for Augsburg Day Student Government, Echo, KAUG, and WEC Student Body Senate officers only.
  
  
• Student Locker Request Form — Students who are interested in having a free locker in Science Building for the entire academic year should complete this form.
  
  
• Workstation Drawer Key Request Form — Student organizations with workstations in the Auggie's Nest may complete this form to request a key for their cube's drawer.
  

Student Group Registration

• Student Club Registration Packet — This form is for starting a new student club for the 2011-2012 academic year, which is for students who want to start a group and DO NOT need access to funding.
  
  
• Student Group Information Change Form — Use this form to update your student group's information with CAO if it changes mid-academic year, including changes to your officers, advisors, contact information, mission statement, etc.
  
  
• Student Organization Registration Packet — This form is for strating a new student organization for the 2011-2012 academic year. Student organizations are recognized by the Student Government and recieve access to funding.
  

Room, Equipment & Event Reservations

• Christensen / Urness-Mortensen Skyway Window Reservation Form — This form is for reserving a set of windows in the skyway between the Christensen Center and Urness-Mortensen Lobby for advertising an event or activity.
  
  
• Equipment Request Form — This form is for reserving the Karaoke Machine, Popcorn Machine, and/or the Sno Cone Machine.
  
  
• Student Involvement Fair Table Request Form — Student groups can complete this form to reserve a table at the 2012 Spring Student Involvement Fair on Wednesday, February 8 from 2pm-5pm in Christensen Lobby.
  
  
• Student Lounge Reservation Form — This form is for reserving the Student Lounge in Christensen Center for an event.
  

Other

• Student Travel Packet — Student groups going on a trip that is 50+ miles away from Augsburg or is nearby but contains risk (i.e. waterparks, Valleyfair, etc.) must have EACH student who is participating complete and turn in one of these packets BEFORE leaving.
  
  
• Technology Services Request Form — Students groups can complete this form to request technology services such as an Augsburg email, Augsburg website, Moodle account, and/or AugNet folder.
